Forms for school or daycare

1) Please complete as much of the form as
you can prior to emailing it to us.
2) Please include a number where we can
reach you if we need information for the form to be completed.
3) Please include instructions on what you’d
like us to do with the form after it has been completed; i.e.
fax to school or call # when completed.
4) Please allow 2-3 days for forms to be
completed. If your need is more urgent please call our office
@ 651-770-2124 to notify a staff member.

Working at this clinic since 1987 I have seen many children grow
into young adults. I have also witnessed, first hand, this clinic
evolve into the wonderful practice it is today.
Not only do I deal with all the needs of the clinics
and staff, I'm also here for the patients and their parents.
I'm available to our parents to discuss special needs of their
